THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Body to Soul Health, LLC is committed to protecting the privacy and security of your health information.
This Notice describes how we may use and disclose protected health information about you, your rights regarding that information, and our responsibilities under applicable privacy laws.
Your Rights
You have certain rights regarding your health information.
Get a Copy of Your Health Record
You may ask to see or obtain an electronic or paper copy of your medical record and other health information maintained by us.
We generally will provide access or a copy within the time required by law. We may charge a reasonable, cost-based fee when permitted.
Ask Us to Correct Your Health Record
You may ask us to correct health information that you believe is incorrect or incomplete.
We may deny the request in certain circumstances, but if we do, we will explain the reason in writing as required by law.
Request Confidential Communications
You may ask us to contact you in a specific way or at a different address or telephone number.
We will accommodate reasonable requests.
Ask Us to Limit What We Use or Share
You may ask us not to use or disclose certain health information for treatment, payment, or health care operations.
We are not always required to agree to such requests.
If you pay for a health care service or item completely out of pocket, you may ask us not to disclose information about that service to your health insurer for payment or health care operations. We will honor that request unless disclosure is required by law.
Receive an Accounting of Certain Disclosures
You may request a list of certain disclosures of your health information made during the six years prior to your request.
The accounting does not include certain disclosures, including many disclosures made for treatment, payment, health care operations, or disclosures you specifically authorized.

You may request a paper or electronic copy of this Notice at any time.
Choose Someone to Act for You
If another person has legal authority to act for you, such as a legal guardian, health care surrogate, or person holding an appropriate power of attorney, that person may exercise your health information rights as permitted by law.
We may verify that person’s authority before taking action.
File a Complaint
You may contact Body to Soul Health if you believe your privacy rights have been violated.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
We will not retaliate against you for filing a complaint.
Your Choices
For certain health information, you may tell us your preferences about how information is shared.
For example, you may tell us whether we may share relevant information with family members, close friends, caregivers, or others involved in your care or payment for your care.
If you are unable to tell us your preference, we may use professional judgment and applicable law to determine whether disclosure is in your best interests or necessary to address a serious and imminent threat to health or safety.
We generally will obtain your written authorization before using or disclosing your health information for purposes that require authorization under law, including certain marketing activities, the sale of protected health information, and most uses of psychotherapy notes if such records exist.
You may revoke an authorization in writing at any time, except to the extent that we have already relied upon it.
How We Typically Use and Share Health Information
Treatment
We may use and disclose your health information to provide, coordinate, or manage your care.
For example, with appropriate authority, a Registered Dietitian may communicate with another health professional involved in your care.
Payment
We may use and disclose health information to bill for services and obtain payment from health plans or other payers.
For example, information may be submitted to your insurance company to process a nutrition services claim.
Health Care Operations
We may use and disclose health information to operate our practice, coordinate services, improve quality, train staff, conduct appropriate administrative activities, maintain records, and contact you regarding your care.
Other Uses and Disclosures Permitted or Required by Law
We may use or disclose health information when permitted or required by law, including for certain public health and safety activities, health oversight activities, workers’ compensation matters, certain law-enforcement requests, judicial or administrative proceedings, research conducted in accordance with applicable legal requirements, organ or tissue donation purposes, coroners or medical examiners, and other legally authorized purposes.
We will comply with applicable federal and state laws that provide greater privacy protection when those laws apply.
To the extent we maintain substance use disorder patient records that are subject to 42 CFR Part 2, those records receive additional legal protections. Such records generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against an individual without written consent or appropriate legal authorization as required by law.
Our Responsibilities
We are required by law to maintain the privacy and security of protected health information.

We must follow the privacy practices described in the Notice currently in effect.
We will notify affected individuals as required by law if a breach occurs that may have compromised the privacy or security of protected health information.
We will not use or disclose your health information other than as permitted by law, described in this Notice, or authorized by you in writing.
Electronic Communications and Client Portal
Body to Soul Health may provide access to secure electronic systems or a client portal for appointments, records, forms, messages, and other health care communications.
When available, clients should use approved secure methods for communications containing sensitive health information rather than ordinary email, website contact forms, or social media messages.
